Adult DayCare
Adult DayCare is designed to meet the needs of people with disabilities or other special needs for shorter periods of time. The services are offered in a protective setting during any part of the day, but less than twenty-four hours per day. Weekend and evening care is also available, please make arrangements in advanced if you are interested in Adult DayCare after hours. The Caring Hearts Program provides nursing home care for residents who have a mental health and/or mental retardation diagnosis who also require long term care services.

Hospice is a program designed to provide comfort and support to patients and their families when a life-limiting illness no longer responds to cure oriented treatments. Provided by staff and volunteers, hospice offers specialized medical care, addressing all symptoms of a disease with emphasis on controlling a patient's pain and discomfort.
Hospice does not prolong life nor hasten death. The social, spirtual and emotional impact of the disease on the patient's family and friends is dealt with. Bereavement and counseling services are offered to families before and after the death of a patient.

Infectious diseases can be minimized or eliminated with proper Infection Control Protocol. A coordinated multidisciplinary approach is required to prevent problems.

IV Therapy
IV or intravenous therapy is delivered to patients in hospitals, long term care facilities, out patient facilities and in the home. IV therapy may be prescribed for people who are dehydrated, who need antibiotic therapy, who are unable to eat regular food and need a special nutritional formula or for other medical conditions. The services is delivered by nurses under RN supervision.

Park Plaza Nursing Home offers on-site rehabilitation, including Occupational Therapy, Physical Therapy and Speech Therapy. Residents are identified for therapy by either physcian referral, at the request for screening by a family member or as assessed for screening by nursing.
Therapy helps the residents to regain independence, confidence and resume activities of daily living with minimal to no assistance.
The Rehab teams also offer therapy for those in transition from the hospital to independent or assisted living at home.

Secure Care
Secure Care units are specialized facilities or wings within an existing nursing home which provide residential care for individuals with dementia, or other related disorders.
Secure Care units are designed to offer an environment which is both calming and reassuring to individuals with memory loss and other related behavioral problems. Special needs of these individuals are addressed and secure door systems guard against wandering. An enclosed, safe outside area allows residents to enjoy the outdoors. Dining and activity rooms are also featured within the confines of this unit.
Staff of the secure care unit utilizes a behavior management program in order to improve the residents quality of life through therapeutic interventions. Family support group meetings are held a minimum of quarterly to assist in providing education and emotional support to family members.

Wander Guard
The main purpose of the Wander Guard System is to protect and guard residents. Wander Guard improves the quality of life for the residents by granting them the maximum possible freedom of movement without compromising their safety.
Residents who are still very mobile and at risk of wandering out of the facility alone, but who would not be safe if they did so, wear a lightweight signaling device on their wrist or ankle. The device is a small bracelet lighter than a wrist watch. Whenever one of the residents protected by Wander Guard is about to leave the facility, the staff is notified by an audible sound and thus able to assist the person. The system's audible alarm can only be cancelled at the door where the resident is attempting to leave unaccompanied.
The benefit of using the Wander Guard System is to encourage the staff to concentrate on the individual needs of all residents, without making any of them feel constantly monitored, and without turning the facility into a secluded area.

Screening to find those residents who are at high risk for wounds is the first step for preventive measures. Routine head-to-toe assessments assist the clinical team in prevention and promote healing.
Residents' physicians and treatment nurses, along with the other clinicians on the multidisciplinary team work with the resident and their families on a treatment plan.
