Payor Sources
Medicare
Medicare provides health insurance for approximately 39 million Americans. This includes people age 65 and over, people with permanent kidney failure and certain people with disabilities. More information can be found on the Medicare Web Site or call toll free, 1-800-MEDICARE (1-800-633-4227). Park Plaza Nursing Home accepts Medicare Insurance once benefit verification is received.
Medicaid
Medicaid is a jointly-funded, Federal-State health insurance program for certain low-income and needy people. Those covered include children, the aged, blind, and/or disabled, and people who are eligible to receive cash assistance payments. Visit the Medicaid Web Site for more information. Park Plaza Nursing Home accepts Medicaid Insurance, once benefit verification is received.
VA Insurance
Recognizing the growing need for Long Term Care for Veterans, the Veterans Administration has set up a variety of community-based solutions to ease the demand. One of these solutions is to form contracts with local nursing homes to provide for these Long Term Care needs when a Veteran's home is too far from established Veteran Facilities. Park Plaza Nursing Home accepts VA contracts. Verification of benefits is required.
Private Insurance
Some private insurances pay for Long Term Care under special circumstances. There are also Long Term Care supplemental insurances available. Azalea Trail Nursing and Rehabilitation Center accepts all private insurances, once benefit verification has been obtained.

An agreement may be made for Private Payment prior to admission to our facility. Please contact the administrator of Park Plaza Nursing Home for more information.
